THE FOUNDATION REQUIRES THAT PROPOSALS BE SUBMITTED IN HARD COPY.

1. Farvue Foundation cover page (required form)

2. One or two page summary of the proposal for which support is requested

3. Organization budget and if applicable, project budget 

4. Total amount of funding needed for the project if not requesting general support

5. Amount requested from the Farvue Foundation

6. Anticipated duration of project

7. Listing of other sources of corporate and foundation grant support, detailing amount secured and requested for the project and for the organization (can use this form if desired). No need to provide more than 20.  

8. Recent audited financial statements (if no financial statements are available, please provide a copy of your most recent Form 990) -- feel free to send us a link or direct us to an online version if available (no need to print out a hard copy as these can be very lengthy) 

9. IRS letter certifying tax-exempt status

10. List of Board of Directors and Officers
